Tips for First Time Patients
For first time patients, these tips will help you obtain the maximum benefit from your visit with us:
- Bring any prior original mammogram or ultrasound films with you. To obtain films, contact the facility where they were performed and request if you can pick them up. You should give the facility at least a week's notice so that they have time to locate your films.
- Request that the reports of your prior visit be mailed or faxed to us. Print out the request form, complete it and mail or fax to the facility or facilities you have visited in the past. Requesting the reports as soon as possible will help get them to us before your visit. Our fax number is 256-536-3554 .
- Bring your insurance card. We will need to make a copy for your medical records.
- You will be asked to remove your clothing from the waist up for the examination; if you wear a top with a skirt or pants, changing will be easier for you.
